A Retrospective Study on Hackett-Hemwall Dextrose Prolotherapy for Chronic Hip Pain at an Outpatient Charity Clinic in Rural Illinois

Objective: To investigate the outcomes of patients undergoing Hackett-Hemwall dextrose Prolotherapy treatment for chronic hip pain.
Design: Sixty-one patients, representing 94 hips who had been in pain an average of 63 months, were treated quarterly with Hackett-Hemwall dextrose Prolotherapy. This included a subset of 20 patients who were told by their medical doctor(s) that there were no other treatment options for their pain and a subset of eight patients who were told by their doctor(s) that surgery was their only option. Patients were contacted an average of 19 months following their last Prolotherapy session and asked questions regarding their levels of pain, physical and psychological symptoms and activities of daily living, before and after their last Prolotherapy treatment.
Results: In these 94 hips, pain levels decreased from 7.0 to 2.4 after Prolotherapy; 89% experienced more than 50% of pain relief with Prolotherapy; more than 84% showed improvements in walking and exercise ability, anxiety, depression and overall disability; 54% were able to completely stop taking pain medications. The decrease in pain reached statistical significance at the p<.0001 for the 94 hips, including the subset of patients who were told there was no other treatment options for their pain and those who were told surgery was their only treatment option.
Conclusion: In this retrospective study on the use of Hackett-Hemwall dextrose Prolotherapy, patients who presented with over five years of unresolved hip pain were shown to improve their pain, stiffness, range of motion, and quality of life measures even 19 months subsequent to their last Prolotherapy session. This pilot study shows that Prolotherapy is a treatment that should be considered and further studied for people suffering with unresolved hip pain.

Patient uses Prolotherapy for 5 body parts over 5 years
AJ is a 57 year old active woman who is a school teacher, loves to walk her dogs, and spend time with her family. She is no stranger to Prolotherapy. In the past four years Prolotherapy has provided chronic pain relief in her shoulder, hand, knee, buttock, and, most recently, her foot. When she first came to see us in 2007 she was having severe shoulder pain. Her doctor had recommended surgery but she was skeptical and went in search of another option. Foot Pain treated with Prolotherapy
Injury strikes again: Her most recent pain was on the top part of her foot. It was sporadic pain for a few weeks and then turned constant. She also picked up the recommended supplements and began using toe stretchers at home. Again, one treatment was all she needed.

● Bone Marrow Prolotherapy – Hip
One of our first patients to receive Bone Marrow Prolotherapy to his hip has reported great results thus far (it’s only been a week and a half) but he says he can feel a difference. MG is a 46 year old contractor who does a lot of physical work. He received regular Hackett-Hemwall Prolotherapy with HGH to his severely arthritic hip from late 2007 to early 2009. He was much improved from this Prolotherapy but had not been able to return to full athletics because the increased activity would aggravate his hip. He was interested in trying out Bone Marrow Prolotherapy.

● Knee Arthroscopy
The Patient thought her arthroscopy was for meniscal repair, but she received a resection – the case of Brenda from Boston – Prolotherapy would have been a much better option! Ischial Tuberosity Pain / Hip & Buttocks Pain
When you have pain on the bottom of the buttock, especially when sitting and running, this could be indicative of ischial tuberosity pain. Sometimes a doctor will examine this area, find it to be tender to palpation, and give you a diagnosis of ischial bursitis.

Aspirin and Coumadin
For many years I have been performing Prolotherapy on patients who come to see me already taking blood thinners, including Coumadin and aspirin. Let me first say that in all my years of doing Prolotherapy on patients on Coumadin, I have never had a problem. I do let them know that there is an increased chance of bleeding and/or bruising of course, and on very very rare occasions someone on Coumadin may develop a hematoma (where blood accumulates in the muscle), but I have never actually had a patient develop a hematoma. Even if it did happen, the normal treatment is to apply heat to it and wait until it resolves.
In regard to aspirin, most folks who take this for blood thinning are taking a baby aspirin or 81mg per day. This amount of aspirin can thin the blood and I have not found it strong enough to inhibit the Prolotherapy. So if you are on aspirin for heart disease prevention, then by all means continue taking it and don’t worry about it inhibiting the Prolotherapy.

● Is there an age limit to getting Prolotherapy?
Losing the ability to be mobile and active is possibly the worst thing that can happen to people as they age. Activity truly keeps the blood flowing. Joints like the hips and knees depend on walking and weight-bearing activities to provide nourishment to the joint cartilage. No walking, no nourishment. No nourishment, no cartilage. No cartilage, no movement. Walking keeps people alive and keeps the body functioning It stiffness sets in, the grave may follow.
Why not treat all patients with Platelet Rich Plasma? Here is my bottom line related to joint instability and PRP. If you have joint instability, you can receive PRP Prolotherapy to the joint a hundred times and you will continue to have instability if ligament laxity (weakness) is involved.

● Hip Osteoarthritis Prolotherapy is a great option for the athlete who wishes to quickly heal an injury and return to the sport that they love. ● Severely Degenerated Hip and a Fluoroscopically Guided Steroid Injection
Mark came into the clinic with a two year history of bilateral hip pain, right hip worse than the left. The pain started two years ago during a run.
Mark typically ran 28-30 miles per week, but had not run for about two years, since the hip pain started. As a 51-year-old, he was getting pretty discouraged because he had tried physiotherapy, chiropractic care, electrical stimulation, laser treatments, and all the recommendations given to him pointed him toward bilateral hip replacement.
